Revive – Home Repair Grant

Your funds should be used for necessary eligible home/property improvements listed below. All repairs must be in similar value, like, and style to the home.

HV AC systems and ductwork	Siding

Windows	Exterior doors (including overhead garage)
Soffit and Fascia	Insulation or weatherization
Sump-pump systems	Ceiling and drywall repair (due to roof failure)

 

Accessibility & Safety Repairs
Entry ramps	Entry steps/stairs
Mold and/or mildew remediation	Levered door handles
Interior/exterior handrails	Widened doorways
Internal chair or wheelchair lifts	Toilet (ADA only)
Relocation of washer/dryer from basement to main level of the home	Shower Modification/ Grab Bars

    
If work is being performed by a related party to the homeowner, the Member must ensure that all repairs included in the original bid are being completed and funds are being used for said repairs. 

• Your total household income must be at or below 80% of Area Median Income (AMI) based on household size according to HUD Income Limits.
• You own the property as your primary residence, and it is one of the following property types: townhouse, condominium, owner-occupied 1-4 family properties, cooperative, manufactured home deeded as real estate and affixed to a permanent foundation.
• You are up to date with all payments on your mortgage and any property tax obligations.
• You must work through a participating FHLBank Indianapolis Member institution (Peoples Bank will serve as your member institution).
• You must identify contractor(s) being used for repairs prior to being considered to receive grant funds.
• Recipients cannot receive funds for the same property within a five-year period, or receive multiple bank grants at any time, unless prior grants that require repayment are repaid or retention period has expired. 
• Applicants may not be students or part-time students who otherwise would have a significant increase in income. Some exceptions may be made. Documentation will be required. 

• State issued Identification or Driver’s License
• Revive Application - must include First and Last Name, Date of Birth and Social Security number of all household members
• Pay stubs representing the last 45 days for all occupants of the household. Pay stubs must reflect year-to-date (YTD) and gross income.
• FHLBI Zero Income Affidavit - if required
• Proof of home insurance - Declaration page
• Deed - Proof of homeownership or recorded mortgage 
• Mortgage statement (showing current on existing mortgage)
• Proof of current paid property taxes if not escrowed into the mortgage
• Contractor Selection Confirmation Form (CSC)
• Two comparable, detailed, itemized contractor bids
• Before Pictures, must be submitted at time of application
• After Pictures will be required upon completion

• Check to see if you meet qualifications for the Revive Grant. (See above)
• Gather and ensure you have all required documents, including two (2) independent contractor bids, for the application. (See above)
• Application packets will be made available on the Peoples Bank website on April 1st, 2025. Fill out the Revive Grant application, this should be completed as soon as possible. Incomplete applications are not accepted.
• If members of your household are over 18 and have zero income, fill out the Certification of Zero Income Sheet.
• Complete the Revive Grant Checklist ensuring all documentation and information is present and accurate.
• Contact Diane Dalton at (219) 853-7589 or Kellie Taillon at (219) 844-4500 to discuss the completed application and schedule an appointment to bring in ALL documents.
• After the appointment, you will not hear from Diane or Kellie until the application is approved.
• Once it is approved, and funds are at Peoples Bank, you will need to notify the contractor that they can begin work and meet with Diane Dalton to sign the initial check payable to the homeowner and the contractor. The amount of this check is half the cost of the project. They will let you know when they can start the project.
• Once the work is completed, you will meet with Diane Dalton to sign the closing documents, and the check made payable to the contractor and the homeowner for the second half of the project's cost.
