When applying for a PPP loan for your business, you will need to submit the following documents:

PPP Documentation

First Draw PPP Loan
  • Completed and signed PPP Application – SBA Form 2483
  • 2019 or 2020 IRS Forms 940, 941, 944, and/or Form 1099-MISC (all that apply), if NOT self-employed
  • 2019 or 2020 IRS Form W-3, if NOT self-employed
  • 2019 or 2020 Schedule C, if self-employed
  • Payroll processor records for all employees, including information documenting employees whose annual salaries exceed $100,000
  • Borrower prepared loan amount calculation
  • Most recent filed Federal Annual Tax Return
  • Articles of Incorporation and Certificate of Existence/Good Standing
Second Draw PPP Loan
  • Completed and signed PPP Application – SBA Form 2483 SD
  • Documentation that substantiates a 25% reductions in revenues as follows:
    • Federal Tax Returns for 2019 and 2020, if opting to use an annual reduction or;
    • Quarterly financial statements for the same quarter of 2019 and 2020
    • Evidence of First Draw PPP Loan usage, in full
  • If you opt to use a different payroll calculation for your Second Draw PPP Loan, you are required to submit the following in addition to the above listed items:
    • 2019 or 2020 IRS Forms 940, 941, 944, and/or Form 1099-MISC (all that apply), if NOT self-employed
    • 2019 or 2020 IRS Form W-3, if NOT self-employed
    • 2019 or 2020 Schedule C, if self-employed
    • Payroll processor records for all employees, including information documenting employees whose annual salaries exceed $100,000
    • Borrower prepared loan amount calculation
    • Most recent filed Federal Annual Tax Return
    • If your First Draw PPP Loan was not funded by Peoples Bank, you will need to submit Articles of Incorporation and Certificate of Existence/Good Standing
